Q: Donjoy arch rival inserts in men's size b for size 11 shoe? do not see this size in your ads.

Many of their ads for one size only. You have to scroll/search to find other sizes.

Q: Do you use these in place of the insert that comes with your shoes or on top of the original insert?

You use them in place of the inserts already in the shoe. But when you take the inserts out that came with the shoes, you may need them to use as a template to cut the DonJoys down to size to allow them to fit. I initially bought these because my orthopedist recommended them to help correct the way I walk, but even the smallest size doesn't fit my shoes properly (WAY too big) so I needed to cut them to get them to fit. While they do help my walk, I'll never get them again. They're expensive, and for the amount I spent for them, I think they should fit without having to take a knife to them.

Q: Can you wear these with a Rigid Carbon Insole underneath it?

I wouldn’t. It would take up too much room in the shoe.
