Q: Should the cat be able to stand up?

My 15lb. Cat is super comfortable in this carrier. She can stand but it is more cozy than other carriers. She likes to be able to look out.

Q: I received the carrier today. What is the purpose of the inside strap (17.5 in in length with a hook) which attaches to the inside bottom?

I think it should attach to your dog/cat collar (if there is no other inside strap)

Q: will this hold my 20 pound dog?

It holds my 16-pound cat, but it's not made for carrying heavy pets for long distances. If your dog is mellow, it will probably work for you. But if he's going to have a fit, then I'd go for something heavier and sturdier.

I don’t know what extensions you are referring to. I see no reference in the product description. I couldn’t find anything on the carrier itself that could be described as an extension.
