Q: How does it keep things cool?

It has a layer of insulating material on the inside and a zip round the top. It will keep sandwiches, yoghurt, canned or bottled drinks etc cool for a few hours.

Q: Do you need to use ice packs to keep things cool or does it automatically do this?

With an average packed lunch there wouldn't be enough room for an ice pack. There's a good layer of insulation inside the bag, and when I pack sandwiches, yoghurt, bottle of water etc at 8am they are still fresh at 1pm. Hope this helps.
