Q: What's the difference between mw2500w and the mw2600w?

They are actually different sizes. If you look at the dimensions in the listing, the 2600W is slightly larger than the 2500W

Q: How would I know if I need the grounded outlet cover?

This case is not grounded, it's just a cover that goes over the existing outlet....and there is no electrical requirements

One of the covers is bigger than the other. I'm guessing the product dimensions are got the smaller one. The one with an inner rectangle is bigger

I doubt you will find an outlet cover with 2 screw holes. Typically there is only one screw hole for an outlet cover. I did not search for 2 screw holes.
