Has anyone used this with the iPhone adapter?

I have not as I have a samsung, which I know doesn't answer your question. However, I have used these headphones, and while they are comfortable and you can wear them when sleeping, they DO NOT last very long. I've gone thru 2 pairs and they've each lasted only about 2-3 months before they stopped working. I won't be buying a third pair.

Are these true noise cancellation headphones? the description indicates it cancels noise via the in-ear design not via actual cancellation technology.

No. Not even close. The physical nature of them is a bit noise cancelling, but they do not provide active feedback loop style noise cancelling like say Microsoft Surface Headphones or others do.

Is there a contact phone number for Maxrock? This is my second pair within 18 months-both stopped working and Amazon was no help resolving my problem

Sorry, I do not have a number for you. I wear my earbuds every night and I blow through them as well. They are cheap enough that I just buy the two pack.
