Q: What is the outer diameter of the plastic piece (not the hole behind it)?

The piece that you see is slightly rubberized and it's about 1/16" shy of 3 3/4". It inserts into the plastic piece that goes in the wall and the outer portion of it is about 3/16th smaller in diameter than the rubberized piece. Of course, the hole is smaller than both of these.

Q: What instrument is used to pass the cables from one end to the next?

I used a washer on the end of a string to drop down from the upper hole and pull the washer out of the bottom hole, then pull the cables down from the hole above.

Q: How do I remove the cover from the grommet? It extremely tight and the grommet lip cracked a bit.

I too found it challenging the first time, then I noticed the nubs holding the inside piece (the flexible cover) and concentrated on making sure I started pushing in at those locations and it slid out easily.
