Q: The packaging says "two pair". are there *really* two pair in there? it looks like just one(?) maybe they peel apart??

Yes, there are two pair (4 pcs total). The halves are stuck on back-to-back so you have to peel them apart. Or if you like lots of cushioning, leave the halves together and put them under the insole... Might feel lumpy though

Q: Do they have any adhesive on one side to keep them from moving around?

One side is sticky. They did not shift in my shoes which were a wedged heel. The packaging says they can be hand washed. I haven't washed mine. So, I do not know how sticky they will be after washing. The same company has another gel insert that's a little larger in size and a little thinner. This thinner one tends to shift in my shoe a bit. I like both products.

Q: The picture I'm looking at is felt covered but from what I'm reading they seem to be just uncovered gel pads. Are they felt covered or just a gel pad?

They are covered, have the feeling of soft cloth - not the "stickiness" of gel. (The gel ones stay in place better, I think.)
