Do you have to use screws to attach it to a door? I dont want to have to put holes in my hardwood door.

No, it is held on by a screw-down clamp, but if your door is not solid it can damage the door. If your door is solid wood or metal it should be OK.

Your neck size isn't really an issue. It will stretch your neck regardless of how small or large it is. Your head tilts back over the stand and your neck is supported. It gives your neck a gentle stretch.

How many lbs of maximum pull traction can be given on this device?

The scale on the traction device shows up to 20 units..Not sure what the units are, as scale does not indicate. The paper instructions may have said what the units are but I don't know where that is...
