Will this fit Horizon organic milk boxes (what my daughter buys at Wegman's)?

Not sure about boxes but they were very big for the smaller pouches we wanted them for.

Are these packaged individually?

No, the ones I got were packaged together. I'll also add, they were way too big for smaller pouches such as Buddy Fruits brand.

Would Apple & Eve brand juice box fit?

Yes they do! They fit perfectly, just be sure to press them in only until the box top aligns with the top of the holder so the straw can still reach all of the juice.

I thought these were made with bottoms now? Mine didn't..

Removable bottoms should have come with it.
