Do I need to pour concrete?

No. A level grass or sand base does it. No slab, no contractor.

What if my yard isn't perfectly flat?

Minor slope is normal. Level the base first; the included manual walks you through it, step by step.

How much upkeep, really?

A few minutes most weeks: skim, test, adjust. That's the whole job.

Is everything included?

Pool, sand filter pump, liner, A-frame ladder, and skimmer. You add filter sand and startup chemicals.
