Are all the output jacks amplified output, or do the outputs on the back provide line-level audio and the phones output on the front is amplified?

Does this have riaa eq or is it a flat preamp?

Unfortunately it’s a flat preamp.

Plugging it from headphone jack on tv to bigger speaker in my van and it sounds bad when i turn it up a little. Any ideas?

It sounds like the output might be too high (amps) even though the sound isn’t that great. You could try to change the combination of settings until you find one that sounds better i.e. turn the tv volume down and the preamp output up or vice a versa until you get a combination that sounds better

What type of pin or plug fits into the ground hole?

There are no dedicated ground 'holes'. ground is either the other ring of the female RCA connectors or the ground position of the stereo minijack (3.5mm).
