No, it does not come in any kind of elaborate case. It is just the pistol alone in a cardboard box.

Yes. I bought this as a toy for my son because he likes to dress up for the revolutionary war. (He is 11)

Is the ramming rod removable?

No, the ram rod is purely aesthetic, and cannot be removed. The feature is nice, as my last denix purchase of a blunderbuss was underwhelming; as it had no ram rod at all. Removable or not, its annoying to be missing a feature that should be there for a supposed "Museum Grade Replica".

What is meant by "left-handed"? Looks like the right thumb would pull the leaver in the image.

I bought this for my Dad and he is right handed. He used his right hand to pull the leaver. The picture shown is what you get, which means it is right-handed. It also has some good weight to it. It's made very well for a prop. Good luck.
