How big around is the barrel of these pens?

Very narrow. Slightly smaller than a standard Bic Pen. And they have flattened sides similar to a yellow school pencil.

It this permanent ink? Will it bleed when wet?

If you color on paper, and the paper gets wet, yes it will bleed. I am not sure what happens if it gets on fabric. I'm guessing it would come out if you used Shout or show a tiny bit if you didn't. But that's a guess! (they are the cutest pens)
