Does it warn you when you need a new battery ? It would be Sod’s law that when I needed it the battery would be flat !

Don't think it warns you, but you can test it now and then by pulling out the pin for the noise (I cover it with a thick pillow to protect my ears) or by activating the torch.

It comes ready to use, you do not have to buy batteries separately.

Can it cause hearing damage?

I really don't know the answer to this, but I would say if you needed to use it to summon help, you shouldn't worry about hearing damage. This might just save your life. I carry mine all the time for peace of mind. I hope this is some help to you.
