Is response time quick enough to avoid lip sync issues?

I haven’t noticed any problems. Seems like they are in sync all the time.

I just bought a new smart 55 inch tv and I find my reciever is too old and does not have a hmdi port. How do I hook it up to my tv for suround?

With this device, you just use your current left and right audio cables to connect to this box which then connects to the tv via an optical cable (included with this box). Audio runs from the tv to this box to your receiver. Works great with our new smart tv and 24 year old receiver.

Does this work for bluetooth headphones and allow you to use the headphones and another person hear the tv with surround sound?

In most cases the answer is NO! If you are connecting to a new smart TV with the digital audio output, the only way to accomplish what you want is to buy a separate sound bar with the outputs you want. Then you don't need this converter. In my case, I had to use this to go to an existing analog sound system to accomplish being able to have both speakers and headphones. It is my contention that new TV's are discriminating against those with hearing issues. I have to buy extra parts so others in the room can hear when I have headphones on. Most new TV's allow only one out put working at a time, unlike the older analog type.

My samsung tv reverts back to dolby instead of pcm each time i turn the tv off/on. any ideas? i'm stumped.

I never experienced anything like that with this converter or any other. Both my Samsung TV stay set independent of the power switch. You might check with Samsung about that.
