Anyone have any luck getting these to work with with Windows 11? Neither of mine work. I get an error in device manager that says to contact supplier

The messed up part is with a little help from Google there is a work around for the PL-2303A chipset with Win 11. AFAIK there is absolutely no reason why Win 11 would work with this chipset. Prolific must have supplied the pseudo driver (the one that lists the error) to Microsoft for use w/ Win 11. Planned obsolescence, BS! I would boycott any product with a Prolific chipset just to put it to them as they try to rip-off consumers. If you already have one, the procedure requires you to delete the nonsense the junk Prolific supplied Microsoft for Win 11 and load the Win 10 driver. I have tried and tested this and it works. DON'T BUY THIS PRODUCT FOR WINDOWS 11 SYSTEMS, native driver will not work and none will be officially available as Prolific has EOL this product in 2019.

Does this operate at rs232 levels or does it only convert to 5v logic levels?

This cable is rs232 signal level. Amazon does sell a similar cable (same chip set) that supports ttl level signal. The other cable has flying leads for your application

Why does that cross-section of the cable show so many wires? shouldn't there be only four lines for usb?

The wires that are shown are for the serial end of the cable. They are shown in groups with 3 tinned copper conductor. the wire groups each go to their connection on the serial end.
