Can i safely use this 5v 12a power supply to power a device that only requires 5v 3a?

the simple answer is yes, the power supply can provide up to 12A. If one device requires only 3A, that's fine. You can have another device(s) that may require 5A. You should not exceed the total 12A. Hope this helps.

There are conflicting pictures on this page. One shows two each of + and - poles, one shows one each. Which one is correct?

The one I received has 2 5V + and two "C" (ground) terminals. They all come off the same rail so it really doesn't matter in the end but, yes, you will like receive the one with 2 each.

Does the 5V 12A power supply require a cooling fan to operate?

The PS does not have an internal fan. It is only a 60W, 5A supply. A separate external fan is not required, if that is question.

Does the fan inside the power supply have pwm/variable speed? or does it always run at the same voltage/rpm?

PHEVOS switching power supply 5V40A/12V20A/12V30A, these are with fans, they operate at the same voltage, they don't change speed.
