What kind of material is?

Strechey comfortable material. 95% modal 5% spandex.

Measuring the waist 38 , What is the right size to buy

I don't know about size 38. Sorry! But medium size is fit very well with me . My size is 31. It's look very sexy underwear.

I wear US medium so i have to buy a Large right?

If you wear a US medium you want at least a large here.

How good is the elastic in the waist band and around the thighs? Is it last-longing or would it become loose after a few initial washes?

As with all my items of this type of fabric I wash them in warm (not hot, but warm) water with basic laundry soap but without ever using a fabric softener. Then I hang them to dry. Treated this way the garment, including the elastic, lasts a long, long time through dozens of wash/wear cycles.
