Q: Can this be as one cover to the foam? Or do I need the foam cover as well?

I'm using it as a secondary cover for another foam mattress (that has its own cover, but that cover should never be removed because it's one of those mattresses that has fiberglass added). Foam mattresses often have fiberglass as cheap flame retardant. If you see shiny fibers or glitter dust, that's probably fiberglass. If you bought your new foam mattress bare and it was exposed already, it's not fiberglass. Also. having two covers makes it far easier to wash the outer one if needed.

Q: will it fit a 10" mattress?

Yes, it will fit up to 18" mattresses.

Q: Will it be loose on a 9" mattress?

The mattress I got was a twin size and expanded to 5 inches and fits a little loose. It's hard to tell if you could squeeze 4 more inches into it! I would say purchase and try it. My grandson loves it! I have washed it 4 times and it comes out wonderful!! It does zipper up to fit the mattress, so it won't slide off!
