*cough* once "broken in" does it squeak like other metal frames?

No squeaking, this frame is pretty sturdy. You may wanna get a rug if you have hardwood floors, it tend to move if you doing extra curricular activities.

Has anyone had trouble with their mattress slipping off the frame?

Little trick I learned a while ago- get some gripping shelf liners (2 for a queen size bed) and roll them out between the mattress and the frame. It will never move again. And if you have a box spring you can put it between the boxspring and the frame and then between the mattress and the box spring. One roll of this should work for two beds or one bed with a box spring. https://www.amazon.com/Duck-Select-Drawer-12-Inch-20-Feet/dp/B002AS9NAS/ref=sr_1_15?ie=UTF8&qid=1536787715&sr=8-15&keywords=dresser+drawer+liner

You can use box spring is you want to, correct?

Hello, Thank you for your question. Yes, you would be able to use a box spring if you would like, however, a box spring is not required. Best Regards, Best Price Mattress Customer Service

My bed came without hardware or instructions. What type of hardware is required?

Full set of custom screws, hex key (a.k.a allen wrench) with instructions is included and supplied with your order. Most likely, the bed you received is NOT a new merchandise. It was a bed sent to someone else and returned by that customer who did not put back the hardware and tools when they re-packed the box when they sent it back. Contact the vendor immediately.
