The description states 0.3 but the pens pictured are 0.03. Which is correct?

I can't speak for other sellers but this item is for 0.3 mm Uni Pin pens. I hope that helps :)

Does the ink smear if you highlight over it?

Only if the ink is wet. If you allow it to dry for a while then no.

The description says you get 3 of the 0.3 pens, but the photos show one each of 0.1, 0.3 and 0.5 pens. Can you please tell me which it is?

I got 3 x 0.3 they are very good I use them a lot.
