Does anyone else find some coffee grounds come over the filter when pressing the plunger down? Otherwise mine might be defective.

This is not a design flaw, or at least not one that cannot be fixed, and nothing to do with the fineness of the coffee. Around the bottom of the push-down filter is a rubber seal. There is a gap in the rubber that can creep around the circumference of the filter during use. It should be positioned directly under the upright plunger stem. If it isn't, grounds will flush up into the coffee by-passing the metal filter screen. The rubber can be gently repositioned if yours comes out of place. I have had my cup for a few years and have corrected this issue a few times.

Does this come with a travel lid?

I've owned two of these, the first one I bought about 5 years ago and it did not come with a lid. However I recently replaced it and it did come with a lid. The lid does not fit the older one so the design appears to have changed slightly but both have served me well.
