If a child doesn’t unbuckle but just stretches the belt loose enough to climb out. Would this help?

Use this then after buckle pull the seatbelt all the way out and let go.this locks the belt so it can not be stretched

Can this be used without a Child Carseat?

Dear customer, Yes, the safety buckle can be used also without a child carseat.

Would this prevent an elderly dementia patient from unbuckling??

Yes, works perfectly for that purpose.

Does it fit snug??? My son is able to put fingers under and still unlock it.

Yes. My daughter and son were not able to unbuckle theirs. It just sits over the buckle itself, and as long as the seatbelt itself is buckled, you should have any problems.
