do they go over the buttons already there or do you have to remove the old buttons ?

On my Tux shirt there are slits beside where the buttons are attached to the shirt. They go through those slits. They would not work for me on a 2nd shirt I have that does not have those slits.

(Preface - I am a sarcastic funny guy...) They come in an elegant plastic bubble wrap bag which is placed inside a deluxe cardboard box (which I suppose could be used as a case). Incidentally, these are my favorite stud and cufflink set. I have no doubt that you will enjoy them, but they do not come in a manufacturer's case.
