Why does the microfiber pad keep coming off the mop when I try to dip it in the water

There are three slots that the mop head slips into on the pad. You have to fit the mop into them And then smooth down the Velcro. If you do that then the pad won’t pull off

Does this work on hardwood floors?

It works great on hardwood floors because you can get most the moisture out of it.
