How do you get the picks out of it? :(

The picks simply slide out of the holder, the upper most one first.

how do you attach it to the guitar

The pick holder has a sticky back which requires a piece of waxed paper to reveal. Once removed the holder can be placed anywhere on you guitar which is convenient.. (The holder will support around five plectrums, not the 10 which come with this offer.). I hope that helps.
