Does this helmet have a hair port?

No, and do keep in mind it is extremely small. It doesn't say it's child size, but it is. It also has minimal ventilation.

Can a 12 year old wear this?

Yes. My 10 year old is wearing it. It has the self adjustable (elastic) fit band.

Can you put a ponytail or bun through the back of this helmet?

No. I have my hair in a pony tail often and have to lower it below the back of the helmet.
