is this a case of four gallon or only one gallon , because previously my friend purchase a case of four and come with 1 gallon ?

They came as a group of (1) gallon containers - 4 total.

Is this the original formula, the ultra formula, or a different formula entirely?

This is the original Professional Manual Pot and Pan product designed specifically for tough business jobs. Long lasting suds clean 58% more dishes vs. private label dish detergents.

Is this price for 1 gallon or for the whole box of 4 gallon?

This is 4 - 1 gallon each bottles.
