This bra has pockets for the insertion of a prosthesis or other type of pads or stuffing. I wear this bra every day. It does not ride up and is very comfortable.

Q: what size do I get if I am 40DD?

Well I am on the other side of the spectrum at barely a double A, But I do find that I like a looser band just to help me breathe so I get the 38 even though I only measure 34 to 36. But I don't have the weight That plays into it. If it's free return and you can purchase two sizes at once I would try that... And I also read the reviews to see if I get any clues because in any case returning is a nuisance. Good luck

Q: does the bottom roll up/bunch up when sleeping

I don't wear it sleeping, so I can't directly answer the question. However, the upper hooks frequently come undone during the day while I'm busy. Fortunately, the bottom one has held together. I haven't had a problem with its rolling up.
