Is it possible to have independent volume on the 3 outputs (1/4", XLR, and 1/8" phones) ?

Don’t know. Didn’t not like the sound of the unit at all and sent it back very quickly.

I got one and it’s super noisy. Is there a way to fix this?

There must be something wrong with your unit, because mine is super quiet. So quiet that it was one of the first things I noticed and liked about it. And it’s the same whether I use headphones or connect it to an amplifier.
