it's not true to size. Go to an AE store and use that Brannock device and try some on. Sizing is nothing like what you wear with sneakers. They run 1-1.5 size larger and narrower than your sneakers. For ex., I wear a 9 men and 7.4 X-wide in Adidas/Nike and AE respectively.

Are these firsts or seconds quality?

I'm not exactly sure what you mean by "firsts or seconds quality". I'm going to answer the question as if you are asking whether they are what you would find at the retail company store, or at an outlet. As stated earlier the craftsmanship is very apparent (my other dress shoes are Johnston and Murphy so I'm going off of that standard) and I took a very close look at the shoes inside and out before trying them on. The shoes were in perfect condition so I'd say they are of firsts-quality.

I don't understand the size chart, what is the difference between a size 8.5 b(n) and an 8.5 2e?

B or b(n) is two times narrower than A standard D size or Medium (normal). A 2E, is on the other end wider than D by the power of 2. These are on the true to a bit narrow for my likes. So order accordingly. Beautiful shoe/nice leather. You do get what you pay. Look at "To Boot New York" brand for a step above this but little narrower also good buy for your money.

I have very flat feet and will need to remove the insole and replace it with one that better accommodates my foot. Can the insole be removed?

No, the insole doesn’t come out. It appears to be glued in.
