Which holes do you measure for sizing, the hole in the ceiling of the hole in the downlight itself?

It will be the size of the hole in the ceiling that you needed to make for the downlight to fit through. So if your downlight needed a 72mm hole then that will be the size of the hole in the bezel you'll need - if that makes sense

Inside hole 27mm outside diameter 50mm

Inside hole is 72 mm. the outer diameter is 112mm, the plastic rim is 24mm on the ones I purchased

is this IP rated? can it be used in bathroom ?

It’s basically a large flat washer that has no effect of the ip rating of the actual downlight.

Are these strong enough to help hold a 1.5kg ceiling light (if added on top of the bulb ring, which itself is too small)?

The item is a decorative bezel & is not intended to bear the weight of a light fitting. It could however be glued to the fitting or ceiling.
