Does it require a Splash mat?

No, a splash mat is not required. I covered mine with river rock.

We don't run our fountain daily. Do you think it would hurt the plastic to use a bit of chlorine in the fountain to prohibit mosquitoe growth?

Thanks for your question. I don't think the plastic would have any trouble with a little chlorine. Just keep in mind that when you do run the fountain it's going to cause that chlorine to dissipate quickly. Chlorine doesn't care for moving water at all.
