Is this price for a 3 pack or for a single item?

These are single items. I ordered 4 sets in July of 2016 based on cmpeiffer's answer that they were sets of 3 and only 4 total came in the box.

What is the outside diameter of theses plugs?

They fit inside the 1 3/4" anchors.
