Does this require two motherboard sata ports for both drives, or just one?

Separate sata port for each drive on the motherboard or on additional sata ports you may have installed in the desktop

Does it come with the fan?

Yes, it does come with a 80mm fan in the rear. I highly suggest reading and looking at the provided images on the product page thoroughly

does this support sata111

The MB971SPO-B supports SATA III (SATA 3) speeds.

Yes. The SATA connectors are pass-thru type on this device, unlike many drive docks. I installed this device solely for system imaging backup/restoration and confirmed my restored drive is bootable. However, do note that the compactness of the 2-drive designs means the HDD can get toastily hot! Even with only 1 x 3.5in HDD it can get very toasty, but a single 2.5in SSD will be perfectly fine ...
