does the rope go around the shoulder of the people lifting?

No. Rope straps the sitting person for added support so that they don't fall forward. Just an added note, I do not recommend using this on a wheelchair. It can slip forward if the patient cannot sit upright on their own.

What is the length of the waist belt?

The waist belt and leg belt both are 18-31 inches. The sides and back make the waist almost 60 inches and the leg about 48 inches. My wife healed faster than expected so we never used it, but seems well made.
