Is this supposed to come with a nipple? Mine didn't.

It is only suppose to come with a spoon attachment. You have to buy the nipple attachment separately for $4.99

Does the food leak out? I see it has a food seal cover but do you have to take it out before feeding baby or do you twist it to allow food to come out

Food leaks out if u dont put the seal In it while traveling , which is kinda inconvienent . no twist , just squeeze

Can it be used for baby food?

Ah...yeah... It is made for baby food. Puréed food.
