Do they work for men’s size 12 usa

Yes, size is not an issue as this will fit under your heel. Works in my 11 1/2 boots with no problem. I used the main insert with 1 add-on. The others work , however, they are a dead give away as this added almost 5 inches to my height. So I went went subtle. Every one thought it was just the Timberlands, shhhh.

Can you workout while having them on or while play all 4 quarters of a basketball game?

It’s possible yes, but it really depends on the shoe you’re wearing, the insoles can’t be too big or there won’t be room for your feet. You also need it to make sure you ankle has support or the shoe is hightop because you could very badly injury yourself.

Can I wear them when I play sports

I don't know, but keep in mind the raise your ankle higher so if your not wearing high tops or over the ankle shoes/cleats for your sport you ankle will roll easier because it doesn't have the support. They are comfortable enough for it.
