Is this the one with "Push down and turn" cap? I dont want a young child to accidentally open use.

Yes it is. I have bought this product from various sources thru Amazon and they have all had the push down and turn cap.

Why did the formula change (Menthol now 16% , was 25%)?

Improve the quality, so that hygiene is not harmful to the body.

how often do you use it?

I only use it when my muscles are score but I knew a women 91 yrs old that used it daily to relieve joint pain. My whole family has it in their medicine cabinets to use for pulled muscles.

Does it have an expiration or use by date?

Yes, the expiration date seems to be about 5 years. I purchased mine in Dec, 2014 and it expires in July 2019. But I'd still use it looked or smelled different that when first purchased.
