will it fit a bar of 22.21 mm

Based in measurements on my bike, it would seem to fit. Worst case, order it, try it out, and if it doesn’t fit, return it. If it worked out, fantastic. If not, you are only out a few minutes w/ the install / deinstall.

Could you make this into a ring for a finger to use as a running bell?

No it is too large for that and it would have to be so tight it would cut your circulation off, it won't even ring if it is not mounted right, the bell part is suspended by little springs.

My bike is 9/10 “ or 22.85 mm, which appears to be between sizes. Is there much wiggle room on the small? Or extra rubber spacers included w/ the Lg?

If I were you, I'd go with the Small. The bell is secured with a screw . Your bar is less than a MM away from perfect fit. Having that screw loose by such a small margin shouldn't affect anything.
