it's been a day and the pad is still really flat, how do i get it to be fluffy? it doesn't look anything like the photo shown.

As a buyer.. You may of gotten a pad one. Try putting in your dryer by it self. Then try and fluff again. Mine is nice but agree not as thick in picture

Can this fit a hospital bed?

I would imagine so. I put mine on a full size pillow top and there's still room so I'll be adding another memory foam pad. I would imagine this cover would also fit a full hospital bed as well.

Can I use the twin xl on a twin bed?

It would have a bunch of excess material, but if you get an additional foam pad, the excess material would get taken up by the added pad and probably make for a very comfy sleep!
