Are the 5lb grip weights? The picture shows standard without grip holes.

Mine has 3 elongated holes along the circumference. I guess you could stick your fingers through them if you have small fingers. The weight itself is pretty small already so you probably wouldn't need to use them as grip assistance but you probably could if you wanted to.

would this plate accomodate 1-inch bar?

no it would not. olympic plates fit olympic bars, which are right around 2" wide. you want to shop for standard plates, which will fit your 1" standard barbell. i hope this helps. take care.

Are these usable without a bar? I need these for shoulder physical therapy, but want to make sure they are easy to grip from the hole to outer edge.

The holes for gripping are fairly small and almost sharp at the edges. I can only barely get three fingers and no thumb through. I would recommend small dumbbells or a different plate which would accommodate the hand better. Even plate twist or front plate raises would be uncomfortable, and for physical therapy I would not recommend.
