How does everyone get on with no arrow keys?

Arrow keys are there, but aren't dedicated. So you use Fn+[key] to use arrow keys. Many people with this keyboard may use editors like VIM; where cursor movement is with other keys. If you aren't used to it, it is a learning curve. But once you are used to it, I wouldn't go back to any other keyboard. That is just a personal opinion though.

why does this cost so much?

Quality of the key-caps and general build quality. I have two - one for home and one for work, where I replaced the office provided one. I think it comes down to how frequently and for how long you use the keyboard. Personally, I wouldn't want to go back to other keyboards. They feel too clunky, once you are used to the HHKB. If you are new to it, it would take you some time to get used to it - a learning curve; so I'd be prepared to stick with it.
