Where is the earth wire supposed to go?

The lamp holder itself is double insulated plastic & doesn't not require an earth wire. Earthing is only required for metalwork, if the light fitting the lamp holder is to be installed in has metalwork attach the earth to that metalwork

How are these holders wired? I cannot see what holds the wires in place

The wires need to be stripped back as normal and are then inserted into their holes. The wire is gripped by an internal spring and cannot be easily removed once inserted.
