I’m 57, should I get a xxl or a 3 xl?

If you mean 57 inches get the one listed at 50 -60 inches. i was 59 and it fit good and was very comfortable.

I think about 10”. It is pretty wide. I’m very satisfied with this product. I would recommend it for hernia support and also if you are looking for back support.

Can you wear this right on the skin, or should you wear it over a T-shirt or under shirt first??

Either way, but over a t-shirt is a little cooler, allowing some wicking. Down side is that the binder can slide up and needs periodic repositioning.

I have an umbilical hernia that can not be pushed back in. will this belt still help or will it cause more harm from confinement?

I will try, but I may not be enough help. My hernia is able to be pushed in, I don't do that, I lay down it goes in by itself. My hernia does not bother me, with our without the belt. I wear it mostly as cosmetic, so that a huge lump is not sticking out of my shirt. I use it to hold it in but I feel no discomfort from it. If your hernia does not go in, I'm not sure what this will do unless it feels better to have it confined. IF the belt did not hide my lump, I would not wear it. I hope this helps some. A physician might be help with information.
