Is this the replacement for 12801 kitchen faucet

No, these replaced my bathrooms faucet. However, they may work, as these also worked in my shower faucet as well.

Are these genuine moen cartridges ?

Moen does not have double ring design

Does this include the plastic valve removal tool?

No - I purchased it separately at the same time. I could have done the job without it, but I can also see if the valve was frozen in place, I would have needed the tool to get it undone.

Does this work with the Moen 4800 27000 64000 74000 84000 series bathroom faucets?

this cartridge is for the older style Moen faucets. The current cartdrige is ceramic discs in a plastic container.
