Will this pull the cabinet off the wall?

No for two reasons 1. Just not that much pressure 2. You turn the jar clockwise to loosen the lid which actually pushes the cabinet toward the wall.......having said this I have seen cabinets so poorly attached that most any weight/pressure could pull cabinet away from wall.

What is the extra blade for and how do I install it?

I didn't get an extra blade. Very easy to use. Just adhere the sticky pad to the underside of your upper cabinet and then stick the opener to that. I haven't screwed it to the cabinet and it's fine. This is the best thing I have bought in years. You won't believe you have lived so long without having this. It opens everything!!

I only have one hand. Can this be used with only one hand?
