Q: Why does it say "organic" in the title?

My best guess is the seeds that fill the pillow are raise organically...never sprayed. BEST PILLOW i have ever had and I have a very bad neck!

Q: Does the pillow keep its shape all night? I tend to move and don't want to have to flip or refluff it.

Yes it does keep its shape as you placed and manipulated it ready for sleep. However, sometimes it is necessary to re adjust during the night.

Q: can I use this to support my lower back while I'm sleeping?

Sure, why not? I like this pillow because it provides firm support for me when I sleep on my side. But I also use it to prop myself up in bed. It fills the space at my lower back.

Q: my pillow arrived loose-no cover-in a generic box. Wondering if it's used-should be wrapped in plastic or something?

Your pillow arrived just as mind did ! That was the same as my previous (gift) orders for the pillow arrived. I just love this pillow and used one just like it for the past thirty years! Just put your regular pillowcase over it.
