Are they already connected?

No, you should connect the lights with the adapter by yourself.

Does this accept 110-240v input?

110v only, at least the one I purchased was 110v only. They might other, (240v) models, but I don't know about that.

Will these completely cover a 12x5x5 bush?

The net lights size is 12ft x 5ft, you can compare with your bush.

Could these be wired to run off of 12 volt dc power

Hi, these are 30V adapter string lights, can not run off of 12 volt dc power. If the dc power lower than 24V, the string lights can not be powered on.
