Do all 4 wheels rotate? Does it have 2 strait wheels?

You get two rotating wheels and two straight ones. But, you can install them on the end of your preference. I installed them on the front end (the end without the handle) and it works fine for me.

What is the size and weight of the box it is shipped in?

The cart needs to be assembled, it takes about 20 minutes to do so. So the art is shipped with the 2 surfaces, bottom and top, the 4 legs, the 4 casters and some hardware. The size of the box is the overall siez of the top piece, and I would say the weight was 20 pounds.
